This role is open to candidates located in Arizona, California, or Nevada to align with team scheduling and Pacific Time business hours. This is a high-impact, member-facing role where you’ll partner closely with field providers to keep schedules full, coordinate care for high-risk members, and ensure consistent follow-up across the member journey. A large portion of this role is outbound outreach (approximately 75–80%), focused on engaging members, scheduling visits, and maintaining ongoing relationships—not one-time interactions.
