Job Description

The Direct Care Provider works closely with individuals participating in AA Living Opportunities programs to assist them in achieving and maintaining their goals as outlined in their Individual Service Plans.

Responsibilities:
  • Transportation & Healthcare Support:

    • Transport individuals to medical, dental, social activities, and other appointments.
    • Assist individuals with personal care needs, including bathing, toileting, dressing, and dental care.
    • Coordinate healthcare services and assist in medical needs.
  • Supervision & Daily Living Support:

    • Provide supervision and training during community and recreational outings.
    • Monitor individuals’ physical and emotional well-being, reporting any changes immediately.
    • Assist with the preparation and storage of meals in compliance with regulations and program policies.
  • Safety & Emergency Procedures:

    • Follow all emergency response procedures for medical situations, natural disasters, fires, and behavioral incidents.
    • Assist with the maintenance and sanitation of the home and its surroundings.
  • Documentation & Compliance:

    • Record and maintain data regarding individuals’ health, progress, and incidents in compliance with Title 17, Title 22, and AA Living Opportunities program guidelines.
    • Maintain administrative records, individual files, inventories, and weekly schedules as needed.
  • Advocacy & Communication:

    • Advocate on behalf of individuals, ensuring respectful and effective communication with participants and relevant parties.
    • Understand support systems related to the individual's life and participate in organization-related training and staff meetings.
  • General Responsibilities:

    • Ensure the ongoing care and supervision of individuals with intellectual disabilities and mental health needs according to care contracts.
    • Communicate daily with the direct supervisor regarding concerns, updates, and individual progress.


Qualifications
Required Knowledge, Skills, and Abilities:
  • Prior experience providing direct care and supervision to individuals with intellectual disabilities (preferred).
  • Excellent communication skills with a respectful and positive attitude toward individuals, coworkers, contractors, and home visitors.
  • Ability to complete non-physical crisis intervention training within the first 45 days of employment.
  • Completion of required staff training and a minimum of 12 hours of continuing education annually.
  • Completion of orientation and on-the-job training within the first two weeks of employment.
Minimum Requirements (Title 17 Compliance):
  • Must be at least 18 years old.
  • High school diploma or GED.
  • Health screening and TB clearance.
  • Live scan fingerprint clearance with a clean criminal record.
  • Valid and current California driver's license with a clear driving record.
  • CPR and First Aid certification (can be acquired upon hire).
  • Ability to write in English.
  • Ability to lift up to 10 lbs as part of normal household tasks such as laundry and dishes.
  • Ability to use common household appliances such as a vacuum cleaner, dishwasher, oven, stove, and laundry machines.
  • Availability to work some weekend, evening, and overnight shifts.
Job Requirements (Community Service Associates - Title 17, 58643):
  • Ability to establish and maintain constructive and appropriate personal relationships with individuals.
  • Minimize risks to the health, safety, and well-being of individuals.
  • Perform first aid and CPR and operate 24-hour emergency assistance systems when needed.
  • Achieve the intended results of the services being performed.
  • Maintain current and valid licenses, certificates, or registrations as legally required.
Annual Training Requirements (Title 17, 58652):
  • Recent developments in Supported Living Services (SLS) theory and practice.
  • Policies, procedures, and practices targeted at meeting SLS objectives.
  • Identification of service delivery issues and challenges, along with strategies to address them.
Education & Experience:
  • High school diploma or GED.
Physical Demands:
  • Must be able to lift at least 10 lbs.
  • Must be able to work at job sites without assistance for prolonged periods.
Work Environment:
  • Work is performed in the homes of individuals, which may be diverse and unpredictable.
