Our Hospital Medicine Nurse

Practitioners (NPs) and Physician Assistants (PAs) work collaboratively with physicians, consultants, nurses, case managers, and rehabilitation teams to deliver high-quality, efficient care to hospitalized patients. In this role, the NP/PA will round in the morning, provide cross-coverage for admissions throughout the shift, and, at times, serve as a first responder for rapid response assessments. This is an excellent opportunity to practice at the top of your license in a collegial, supportive environment with a high degree of autonomy.

Key Responsibilities: Perform initial patient evaluations, including history, medication reconciliation, physical exam, diagnostic review, and documentation of H&P with admission orders

Monitor patient progress and coordinate care plans with attending physicians and interdisciplinary teams

Document ongoing care through progress notes

Perform procedures in accordance with clinical privileges

Manage discharges, including medication reconciliation and coordination of follow-up care to support safe transitions

Educate patients and families on diagnoses, medications, procedures, and post-discharge care

Triage and facilitate patient transfers from the CDH Emergency Department, outside hospitals, and outpatient settings

Communicate with accepting providers to ensure seamless transitions of care

We are seeking a part-time (20-hour) NP or PA to join our Hospital Medicine team at the Cooley Dickinson Main Campus (30 Locust St, Northampton, MA). This is a day shift position (7:00 AM - 7:00 PM) with shared weekend and holiday coverage.

Job Summary The Physician Assistant (PA) is a licensed provider. The PA is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation.

Does this position require Patient Care? Yes Essential Functions Provides direct care, counseling, and teaching to a designated patient population in the ambulatory, inpatient, operative, and/or procedural setting.

Performs complete histories and physical examinations.

Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.

Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.

Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.

Consistently provides high quality and timely documentation including admission and progress notes, procedure notes, operative notes and discharge summaries.

Performs bedside procedures as are appropriate to the patient population.
