Position Overview We are seeking an experienced Remote LPN/LVN to support patients enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. In this role, you will conduct scheduled phone consultations, review patient-reported data, document findings, and work with providers to ensure patients receive the right care at the right time. This is a full-time role, Monday–Friday, 900 AM–530 PM EST. Key Responsibilities • Conduct monthly phone consultations with patients to provide education, coaching, and chronic disease support. • Educate patients on condition self-management and encourage adherence to provider care plans. • Review and respond to remote patient monitoring data (e.g., blood pressure, weight, glucose). • Accurately document all interactions and findings in the electronic medical record. • Escalate urgent or emerging patient health concerns per established protocols. • Build lasting relationships that increase patient engagement and adherence. • Meet productivity and quality standards in collaboration with the Nurse Director and Team Leads. Qualifications • Active Nursing License (LPN/LVN) in good standing (compact preferred) • Must reside in one of the following states TX, FL, TN, OH, NM, NJ, GA, VA, NV, MS, or SD • 3+ years of clinical experience in primary care, cardiology, internal medicine, home health, or chronic care management • Strong clinical judgment, patient communication, and problem-solving skills • Proficiency with electronic health records (EHR) and documentation systems • Technical Requirements Must provide your own equipment, including a computer with 8GB RAM minimum, reliable high-speed internet, and a headset • Bilingual skills (Spanish strongly preferred) are a plus but not required What We Offer • $21.00 – $24.00 per hour, based on experience • Paid time off and holiday pay • 100% remote role This is a work-from-home position.
