Part-Time Remote Registered Nurse (RN) for Virtual Patient Care (Bilingual EN/SP Preferred) Job Description Lara Health is excited to offer a dynamic opportunity for a dedicated and skilled Registered Nurse (RN) to become a pivotal part of our innovative Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) team. This part-time, fully remote position is tailored for a professional who thrives in a virtual environment and is ideally fluent in both English and Spanish. The role is designed to provide comprehensive nursing care to patients with chronic health conditions, allowing for effective management and improved patient health outcomes from the comfort of their own home. The ideal candidate will join a robust team committed to enhancing patient care through the effective utilization of digital health tools. This position requires flexibility in scheduling, with a minimum commitment of 25 hours per week, ensuring you can manage personal obligations while achieving professional fulfillment. By integrating your deep clinical expertise with our cutting-edge technology, together we can drive positive health changes across diverse populations. Duties and Responsibilities • Lead and coordinate virtual care efforts among Medical Assistants, LPNs, and other team members. • Serve as a clinical point of contact for escalations and care plan adjustments. • Collaborate with providers to ensure continuity and quality of care across patient touch points. • Monitor team workflows and contribute to improving efficiency and patient outcomes. • Participate in virtual team huddles, case reviews, and care strategy sessions. • Support onboarding and mentorship of new care team members. • Monitor patient vitals (blood pressure, blood sugar, etc.) and medication adherence. • Support patients in scheduling MD visits and making lifestyle changes. • Maintain accurate and up-to-date Electronic Medical Records (EMRs). • Collaborate with providers and care team to optimize patient care. • Perform other duties as assigned to support the team and patient care efforts. Requirements • Licensure Active RN license (mandatory). • Experience Minimum 5 years in nursing, with significant experience in chronic care settings. • Language English and Spanish strongly preferred. • Strong communication, organizational, and time management skills. • Ability to work independently and collaboratively in a remote setting. • Proficiency with computers and familiarity with Electronic Medical Record (EMR) systems; must have own device and reliable internet access. • Familiarity with medical terminology and clerical tasks. • Passion for working with older adults and delivering preventive care. • Preferred Experience Prior experience in care team leadership or practice management, familiarity with remote care coordination tools, and EMR-based task management. • Ability to delegate, prioritize, and manage multiple patient cases simultaneously.
