RN Nurse Manager Rehab and Long-Term Care

LifeBridge Health is seeking an experienced RN Nurse Manager for our Rehab and Long-Term Care services within the Healthcare & Medical Services department. In this leadership role, you will provide day-to-day clinical and operational oversight for a team of nurses and support staff caring for patients who require rehabilitation, restorative services, and long-term care. The RN Nurse Manager is responsible for ensuring high standards of nursing practice, regulatory compliance, and excellent patient outcomes. You will lead and mentor an interdisciplinary team, collaborate closely with physicians, therapists, and case managers, and coordinate individualized care plans that integrate rehabilitation goals with long-term care needs. Your leadership will drive continuous improvement in clinical quality, patient experience, and operational efficiency, reflecting LifeBridge Health’s culture of collaboration, innovation, and continuous learning.

In partnership with senior nursing leadership, you will manage staffing and scheduling to maintain appropriate coverage, support staff engagement, and promote a healthy work environment. You will oversee orientation and ongoing education for nursing staff, facilitate competency development in rehabilitation and geriatric/long-term care, and encourage the adoption of evidence-based practices. You will monitor key performance indicators, including quality metrics, patient safety events, and regulatory readiness, and lead unit- based initiatives to improve outcomes and processes.

This position will work closely with patients, residents, and families to provide clear communication, education, and support, ensuring that care plans are understood and aligned with patient goals and preferences. You will play an essential role in discharge planning and transitions of care, coordinating services and resources to reduce readmissions and promote continuity across settings.
